Clinical Social Worker MSW or LCSW, Home Health (Outpatient), Full-Time, 8:30 AM - 5:00 PM

About the role

The Clinical Social Worker, MSW at PIH Health is responsible for interviewing and evaluating patients and family members to identify social, emotional, and economic factors that may interfere with their medical care. They develop and implement a social work treatment plan, select and utilize community resources, make referrals to other professionals or agencies, provide psycho-social therapy, and participate in multidisciplinary health team conferences.

Responsibilities

• Interviews and evaluates patients and family members to identify social, emotional, and economic factors affecting medical care.
• Develops and implements a social work treatment plan for patients and families.
• Selects and utilizes community resources as part of the treatment plan.
• Makes referrals to and coordinates with other professional disciplines or agencies.
• Provides psycho-social therapy using various treatment methods.
• Participates in multidisciplinary health team conferences to formulate overall patient care plans, interpreting psycho-social aspects of the patient's illness.
• Aids in the coordination of the discharge plan.

Requirements

• Demonstrates skill in patient assessment, intervention, planning, and evaluation.
• Demonstrates initiative, good judgment, and the ability to make decisions independently.
• Demonstrates skill in working with patients experiencing planned and unplanned life transitions, including chronic and acute illness, death and dying, and complex family dynamics.
• Strong communication skills, both verbal and written.
• Ability to learn department and hospital policies and protocols.
• Knowledge of managed care payment requirements and Medi-Cal and Medicare requirements.

Qualifications

• Master’s Degree from an Accredited School of Social Work.
• Current BLS from the American Heart Association (if assigned to Palliative Care, Hospice or Home Health).
• Evidence of continuing education.
• Management will track clinical hours.
• Experience with regulatory requirements related to reporting of child/elder abuse, domestic violence, and other legal requirements.
• Must possess valid, unrestricted California Driver’s License and valid auto liability insurance.
