About the role
The Clinical Social Worker, MSW interviews and evaluates patients and family members to identify social, emotional, and economic factors that may interfere with obtaining maximum benefits from medical care. They develop and implement a social work treatment plan, select and utilize community resources, make referrals to and coordinate with other professional disciplines or agencies, and provide psycho-social therapy using a variety of treatment methods. The role involves participating in multidisciplinary health team conferences to interpret the psycho-social aspects of the patient’s illness and assist in coordinating discharge plans.
Responsibilities
- Interview and evaluate patients and family members to identify social, emotional, and economic factors affecting medical care.
- Develop and implement a social work treatment plan for patients and families.
- Select and utilize community resources as part of the treatment plan.
- Make referrals to and coordinate with other professional disciplines or agencies.
- Provide psycho-social therapy using a variety of treatment methods.
- Participate in multidisciplinary health team conferences to formulate overall patient care plans.
- Interpret psycho-social aspects of the patient’s illness to the healthcare team.
- Assist in coordinating discharge plans.
Requirements
- Master’s Degree from an Accredited School of Social Work.
- Current BLS from the American Heart Association (if assigned to Palliative Care, Hospice, or Home Health).
- Evidence of continuing education.
- Knowledge of managed care payment requirements and Medi-Cal and Medicare requirements.
- Experience with regulatory requirements related to reporting child/elder abuse, domestic violence, and other legal mandates.
- Valid, unrestricted California Driver’s License and valid auto liability insurance.
Skills
- Demonstrates skill in patient assessment, intervention, planning, and evaluation.
- Demonstrates initiative, good judgment, and the ability to make decisions independently.
- Demonstrates skill in working with patients experiencing planned and unplanned life transitions (e.g., chronic/acute illness, death and dying, complex family dynamics).
- Strong verbal, non-verbal, and written communication skills.
- Ability to learn department and hospital policies and protocols.
Preferred Qualifications
- Three or more years of work experience in a healthcare setting.
Pay
$35.00–$57.50 per hour
