Responsibilities

• Provide emotional and logistical support to patients and their loved ones during their final days.
• Affirmatively ensure that patients' financial, funeral, and other arrangements align with their wishes.
• Navigate the grieving process for patients' loved ones, offering guidance and support.
• Engage in both large and small acts of kindness to enhance the quality of life for patients and families.

Requirements

• Masters of Social Work from an accredited school, except those grandfathered under the 2008 Medicare Hospice Condition of Participation.
• At least one year of social work experience in a healthcare setting.
• Experience working with terminally ill patients and their families is preferred.
• License and/or certification required by state minimum requirements, depending on location (CT, DC, DE, FL, GA, IL, NJ, OH, TX, WI).

Qualifications

• Master of Social Work degree.
• One year of relevant social work experience in a healthcare setting.
• Licensed Master Social Worker (LMSW), Licensed Graduate Social Worker (LGSW), Licensed Social Worker (LSW), or equivalent, depending on state requirements.
