Clinical Social Worker
About the role
The Clinical Social Worker I/II works within the Stimulant Treatment Outpatient Program (STOP) by providing substance use disorders services using evidence-based, culturally responsive, and trauma-informed practices adapted for adults with serious mental illness and high psychiatric and/or justice system involvement.
Responsibilities
Psychosocial Assessment: Screens referrals for eligibility and schedules intake or refers clients and referring staff to appropriate resources. Completes client orientation and consents for program services. Assesses presenting problems, psychosocial history, mental status, DSM diagnosis, and level of care needed. Develops problem lists and care plans. Evaluates clients' responses to interventions and progress toward goals, and adjusts care plans as appropriate.
Clinical Interventions: Provides evidence-based, culturally responsive, trauma-informed substance use counseling in group, individual, and family modalities with adults with substance use disorders, serious mental illness, and disruptive behaviors. De-escalates clients when needed. Provides digital navigation support to clients as needed, such as helping them set up devices and data plans, or showing them how to use therapeutic apps. Collects point-of-care urine drug tests according to protocol and documents results. Explains urine drug screen results to clients and supports them in accessing related incentives. Engages clients in substance use disorders treatment and outside recovery resources, coordinates with other organizations, and advocates for clients. Schedules services and reminds clients and collaborating staff. Provides referrals and care coordination for medications for substance use disorders and higher levels of care when needed. Provides targeted case management. Coordinates care and communicates identified mental health, medical, and support needs to clients’ Citywide intensive case managers, psychiatrists, and other service providers as needed. Communicates effectively within multidisciplinary teams within and outside the agency. Ensures that services are received and documented for their caseload. Identifies clients with low engagement or progress and adjusts services to improve outcomes. Conducts outreach and motivational interviewing when needed to re-engage clients. Responds to emergent client needs, utilizing crisis intervention and providing 5150 services as needed. Follows client discharge protocols, including notice of adverse benefits determination and referral to appropriate services. Provides education to Citywide staff, interns, or the community regarding program services, client substance use and treatment needs. If desired and appropriate, supervises students and participates in student training program meetings. Maintains a culturally humble and anti-racist approach. Contributes to professional development of self and others. Adheres to ethical, legal, privacy, safety, health, infection control, resource utilization standards, and other agency policies. Works in multiple community settings, including street outreach, SRO hotels, shelters, hospitals, or jail if necessary.
Administrative Responsibilities: Documents clinical work in an accurate and timely manner. Maintains electronic and paper records according to county, state, and federal requirements. Submits accurate charges for services provided. Meets program productivity standards for billed services. Contributes to quality assurance, including tracking client documentation due and peer reviews of documentation. Contributes to quality improvement, including participation in surveys, interviews, focus groups, quality improvement meetings, and process and outcome reports. Completes required trainings and passes proficiency assessments. Attends meetings and utilizes technical assistance and supervision as assigned. Supports day-to-day facility operations. Other duties as assigned.
Qualifications
CSW I: Master’s degree in social work, psychology or related field from an accredited university, registered or eligible to register with the California Board of Behavioral Sciences (as ASW, AMFT, or APCC), and knowledge and abilities essential to the successful performance of the duties assigned to the position.
CSWII: Master’s degree in social work, psychology or related field from an accredited university and possession of the requirements necessary for application for licensure (as LCSW, LMFT or LPCC) as determined by the California Board of Behavioral Science; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
Strong interpersonal skills that promote effective work and collaboration with individuals of diverse backgrounds in multicultural environments. Clear, well-organized oral and written communication skills. Ability to follow oral and written instructions accurately; detail oriented. Ability to set priorities, work independently, follow up reliably, and efficiently meet deadlines. Ability to work in a high stress area and apply sound judgment when under pressure. Ability to quickly learn, use, and problem-solve, electronic health records, and digital applications and devices. Experience working in behavioral health or medical settings with ability to further learn concepts and terminology pertinent to behavioral health services. Demonstrated skills in evidence-based, culturally responsive, trauma-informed individual and group substance use counseling, with clients with substance use disorders, serious mental illness and disruptive behaviors. One year providing clinical or case management services to adults with serious mental illness and/or substance use disorders. Ability to work safely and efficiently in a variety of environments, including street, low-income housing, jail and hospital settings. Ability to lift 50 lbs and climb stairs. Demonstrated punctual attendance and reliability. Knowledge of ethical, legal, and confidentiality standards pertaining to substance use. Ability to pass background clearance, including for crimes, fraud, or abuse related to federal healthcare programs.
