Responsibilities
- Conduct comprehensive diagnostic assessments
- Develop and implement treatment plans
- Engage in family work
- Collaborate with both internal teams and community providers
- Deliver patient-centered interventions
- Collaborate with other professionals to evaluate patients' medical or physical condition
- Monitor, evaluate, and record client progress according to measurable goals
Requirements
- Master's Degree in Social Work
- Experience in a medical setting (0-1 year required)
- Licensed Clinical Social Worker (LCSW) license upon 1 year of hire if hired as MSW
Qualifications
- Excellent organizational and time management skills
- Excellent crisis intervention skills
- Good problem-solving and conflict resolution skills
- Ability to work well collaboratively and independently
- Strong written and verbal communication skills
- Knowledge of community resources and the aging process
Skills
- Formulation of biopsychosocial assessment, disposition, and treatment plans
