About the role

The incumbent will support both patients admitted to PES as well as carry an outpatient caseload. The incumbent will provide consultation and support to a multidisciplinary team in PES around appropriate resources for safe and effective hospital discharge planning as well as care coordination and treatment referrals to patients on the unit.

Responsibilities

  • Service Provision: Provides ongoing mental health services, including psychotherapy (Individual, Group, & Family), Crisis Intervention, Case Management and Outreach to a caseload of severely mentally ill clients. Makes case terminations and discharge plans, when clinically appropriate.
  • Service Planning, Consultation and Liaison: Develops a comprehensive individual service plan for each client, integrating knowledge of a wide range of human service resources. Provides consultation to a multi-disciplinary treatment team on the psychosocial ramifications of psychiatric illness. Facilitates treatment coordination among various agencies and advocates for and links clients to needed services. Provides consultation to and maintains liaison with campus and community programs, including educational, legal, and social organizations involved in case management. Acts as liaison to inpatient units to facilitate referrals and service treatment.
  • Intake/Assessment: Screens referrals for meeting program criteria; conducts intake of new clients, including assessment, community functioning, and service needs, when appropriate. This entails assessment of presenting problems, psychosocial history, mental status and diagnosis (DSM-V). Responds to unscheduled “walk in” clients, utilizing crisis intervention techniques and providing 5150 services as needed. Serves as weekly OD screening clients for entry into the clinic.
  • Record Keeping and Administration: Maintains client’s records according to California, City and County requirements. Records data in client’s charts. Meets program productivity standards. Assists in processing quality improvement requests. Completes billing and other documentation of services in a timely fashion. Attends staff and clinical team meetings, and participates in training sessions (e.g., In services, grand rounds). Identifies program problems and makes recommendations to supervisory and administrative staff.
  • Education and Training: Provides education to the community regarding program services, client needs and treatment. Assists in planning and providing in-service to staff. Provides orientation and didactic training to interns. If desired and appropriate, supervises student(s) and participates in student training program meetings.

Qualifications

  • CSW I: Master’s degree in social work, psychology or related field from an accredited university and registered with the Board of Behavioral Science as An Associate Clinical Social Worker (ASW) or Intern Marriage Family Therapist (IMFT); and knowledges and abilities essential to the successful performance of the duties assigned to the position.
  • CSWII: Master’s degree in social work, psychology or related field from an accredited university and possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science; and knowledges and abilities essential to the successful performance of the duties assigned to the position.
  • Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities, and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology and psychiatric medication Ability to work collaboratively in a multidisciplinary setting Excellent communication and interpersonal skillsAble to work as a team member in a culturally diverse setting.Able to lift 50lbs and climb stairs.
  • Demonstrated excellent attendance and reliability. A preliminary job offer and assignment may be extended pending successful background clearance. An understanding of mental health issues and sensitivity toward clients with mental health disorders Ability to work with clients and program staff from diverse cultural and social class backgroundsTwo years working with severely mentally ill adults, the homeless, and dual diagnosed populationsOne year experience doing community-based case management and the ability to do outreach including to SRO hotels and shelters Case Management experience
  • Prior work experience or training in an acute hospital settingPrior related UC experience
