Clinical Social Worker 1
Job Summary
This is a per diem, as needed position. Shifts are 8/hr and are M-F, Day shift. A minimum of four (4) shifts per month are required.
About the Role
The Clinical Social Worker I- FLOAT will be responsible for evaluating patients' medical or physical condition and assessing client needs as part of the interdisciplinary team. They will collaborate with other professionals to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the community.
Responsibilities
- Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues.
- Formulates biopsychosocial assessment, disposition, and treatment plans.
- Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
- Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
Qualifications
- Master's Degree in Social Work required.
Requirements
- No experience in lieu of a degree accepted.
- Licensed Clinical Social Worker [LCSW - State License] or Licensed Independent Clinical Social Worker [LICSW - State License].
