Clinical Services Navigator – Acute Palliative Care

Serves as the primary liaison between the patient, family, and medical team while providing patient advocacy and support. The Clinical Services Navigator coordinates care and facilitates timely transitions. This role collaborates closely with providers, care teams, and community partners to help patients and families navigate complex healthcare needs.

Responsibilities

  • Serve as the primary point of contact for hospitalized patients with serious, life-limiting illnesses and their families.
  • Coordinate and manage the daily patient census to ensure timely patient evaluation and follow-up.
  • Triage new consults, assess patient needs, and facilitate access to inpatient palliative care services.
  • Collaborate with physicians, nurse practitioners, social workers, chaplains, and other interdisciplinary team members to coordinate comprehensive patient care.
  • Facilitate advance care planning discussions, helping patients identify and communicate their goals, values, and treatment preferences.
  • Support goals-of-care conversations and assist patients and families in making informed healthcare decisions.
  • Monitor and communicate patient symptom management needs to the palliative care team to ensure timely interventions.
  • Educate patients and families on disease progression, symptom management strategies, and available supportive care resources.
  • Serve as a liaison between patients, families, healthcare providers, and community resources to promote coordinated, patient-centered care.
  • Advocate for patient and family needs while supporting care plans that align with individual goals and preferences.

Requirements

  • 4 Year / Bachelors Degree (Required).
  • Three to five years of nursing experience (required).
  • Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings (preferred).
  • Current nursing license in appropriate state (required).
  • Experience in ICU, Critical Care, or Med/Surg (preferred).
  • Hospice and/or Palliative Care experience (a plus).
  • Strong communication, care coordination, and patient advocacy skills.
  • Ability to work collaboratively with multidisciplinary teams.
  • Ability to successfully complete generic and department-specific skills validation and competency testing.
