Job Responsibilities
- Collaborate with the HTC team to support coordinated patient care
- Coordinate care and communication with providers and pharmacists
- Triage incoming calls for the designated patient population
- Provide patient education tailored to population-specific needs
- Cookordinate annual patient visits, including preparation and distribution of materials
- Assist patients and families with completing paperwork for school and sports participation
- Ensure timely follow-up and continuity of care through effective coordination practices
What We're Looking For
- Education: 4 Year / Bachelors Degree Required.
- Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.
- Licensure/Certification: RN, required.
- Additional Skills/Requirements (required): Ability to successfully complete generic and department-specific skills validation and competency testing.
