Responsibilities
- Coordinates and navigates palliative care for patients.
- Facilitates access to palliative care services in HMH.
- Identifies new outpatients as they are diagnosed with a serious illness and provides patient and family palliative education.
- Connects patients and family with available resources in the health system and community.
- Palliative Care Plan:
- Participates with palliative encounters; provides initial patient needs assessment.
- Facilitates patient's advance care planning discussions.
- Completes controlled substance prior authorizations when needed.
- Facilitates telephone encounters to assess symptoms, palliative medication modifications, transitions of care, provides education and emotional support.
- Documents assessment and follow up, and any encounter notes in the patients' medical record.
- Relationships:
- Buils and maintains professional relationships with health care professionals and provides them education regarding outpatient palliative care services.
- Collaborates with the referring provider/practice to act as a resource for the patient and family during initial or follow up palliative encounters and provides continuity of care.
- Quality:
- Participates in appropriate committees/councils.
- Tracks the flow of patients through palliative care services across the continuum of care.
- Aids in reviewing reports of activity to the clinical leader.
- Palliative Care Conferences:
- Collaborates with the palliative care team to transition patients from other care settings to the outpatient palliative clinic and across various settings.
- Identifies patients for the applicable palliative sub specialty clinics or other palliative services.
- Education:
- As applicable, represents palliative services.
- Provides community organizations with information about palliative services provided and offered to patients.
- May participate in community health fairs, meetings, or programs relevant to the promotion of the program.
- Provides education on palliative care for various team members, orientation, and nursing education, as indicated.
Qualifications
- Graduate of licensed school of nursing; RN with at least 5 years of clinical experience in a relevant clinical specialty.
- Proficient computer skills- proficiency in Google, Microsoft, EPIC.
- Strong organizational, time management, critical thinking RN skills.
- Ability to work independently and exercise sound judgment in interactions with providers, Clinicians, patients, and their families.
- Excellent written and verbal communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
