Our multidisciplinary teams provide culturally-sensitive, end-of-life, and palliative care services to patients and their families at home, in assisted living communities, skilled nursing facilities, and hospitals.
Working on-site at a major medical center, the Clinical Program Liaison evaluates and establishes a plan of care for patients referred for Hospice Care services. As a key member of the business development and marketing team, this role develops and maintains positive relationships with physicians, social workers, case managers, and discharge planners within the medical center.
Responsibilities
- Evaluate and establish plans of care for patients referred for Hospice Care services.
- Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners.
- Coordinate resources and personnel within a comprehensive interdisciplinary team environment.
- Demonstrate effective critical thinking and independent problem-solving skills.
- Work non-judgmentally with patients and family members of diverse cultures, religions, socio-economic backgrounds, and lifestyles.
- Speak in public and work within an integrated healthcare team.
Requirements
- Bachelor’s degree from an accredited School of Social Work; LCSW, MSW, or LMSWA.
- Minimum of two years’ experience in either Medical-Surgical or Community Health.
- Hospice experience preferred.
- Proficiency in Microsoft Word and Excel.
- Ability to work flexible hours to meet the needs of the position.
