Responsibilities
- Provide vital education and information on available options, the informed decision-making process, and realistic goal setting to empower patients/caregivers to actively participate in their plan of care.
- Establish and coordinate a supportive care plan with goals and interventions based on treatment and individual patient/caregiver needs, preferences, beliefs, and values.
- Review, evaluate, and revise the plan of care for appropriateness on an ongoing and timely basis.
- Participate in the development of guidelines, protocols, pathways, and other performance improvement activities related to patient care.
- Work with the Patient Advisory Council to coordinate, implement, and evaluate educational programs/projects for patients and the community.
- Assist in developing a comprehensive diagnostic/treatment plan through interdisciplinary input and attendance at weekly meetings.
- Manage clinical data throughout all phases of patient care from initial referral to follow-up; evaluate clinical data and participate in clinical/patient rounds.
- Develop, coordinate, implement, and evaluate educational programs/projects for patients/caregivers, nursing staff, and support staff.
- Provide educational sessions for patients/caregivers and clinical staff throughout the Medical Center.
- Provide leadership in developing standards of care for patients undergoing new therapies and demonstrate knowledge of approved protocols for specialized nursing care.
- Track volume of referrals and transports, measure outcomes, and work to increase procedures and patient participation in clinical and research programs.
- Recommend, develop, and implement policy/protocol revisions and performance improvement activities based on current standards.
- Assess educational needs in collaboration with nursing and medical personnel and evaluate the effectiveness of educational programs.
- Collaborate throughout the Cedars-Sinai Health System to initiate and implement educational programs.
- Independently coordinate patient programs, projects, and events with Nursing, medical center departments, and communities across the healthcare continuum.
- Serve as a resource and consultant for educational programs throughout the Cedars-Sinai Health System.
- Promote and facilitate research by encouraging inter- and intra-departmental collaboration and utilizing education and clinical research findings to design/revise programs.
- Work to create new transfer agreements with local hospitals and build referral relationships with key physician groups and hospitals in Southern California.
Hybrid remote role: must currently live in the Los Angeles area or be willing to relocate for onsite requirements.
Requirements
- Three or more years of experience in an acute care setting (e.g., transplant, cancer, breast, prenatal).
- Solid understanding of the Nursing Practice Act, The Joint Commission, and other state and local regulatory requirements.
- Preferred experience with program development and implementation of navigator programs.
- Experience with organizational program or project coordination/support, applying program/project management and development procedures.
Qualifications
- Bachelor’s of Science in Nursing (BSN) required; Master’s degree preferred.
- California RN state license required upon hire.
- ANCC Specialty Certification required (in area of specialty) within 1 year of employment.
