Clinical Pharmacist Specialist - Ambulatory Care

About the role

Seeking a Clinical Pharmacist Specialist to develop and support outpatient clinical services, collaborating with an interdisciplinary team to optimize medication therapy, manage chronic disease states, and improve access to specialty medications. This role involves delivering direct patient care, supporting program development, quality initiatives, and serving as a clinical resource for providers and patients. The pharmacist will evaluate patient health, monitor therapy goals, and participate in research, education, and performance improvement activities. Qualifications

• Doctorate in Pharmacy (PharmD)
• 2+ years of experience in medication therapy management, with MO Medication Therapy Services certification
• Strong clinical, communication, and collaborative skills
• Preferred PGY2 residency
• Ability to mentor staff, residents, and students
• Experience in drug use evaluation and cost reduction programs
