Clinical Pharmacist Specialist - Ambulatory Care

Role Overview

This position involves providing expert clinical pharmacy services within an outpatient setting, collaborating with interdisciplinary teams to optimize medication therapy, manage chronic disease states, and enhance access to specialty medications. The pharmacist will support program development, contribute to quality initiatives, and serve as a clinical resource for providers and patients, with a focus on improving patient outcomes and medication management.

Key Responsibilities

• Assess patient health status and medication profiles to ensure optimal outcomes.
• Monitor therapy effectiveness and document activities.
• Participate in professional development, research, and performance improvement activities.
• Educate and mentor pharmacy staff, residents, and students.
• Identify and implement drug cost reduction strategies.

Qualifications & Skills

• Doctorate in Pharmacy (PharmD).
• Minimum of 2 years of experience, preferably with medication therapy services in Missouri.
• Licensed Pharmacist (RPh) with relevant certifications.
• Strong clinical judgment, communication, and collaboration skills.
