Clinical Pharmacist - per diem

About the role

The clinical pharmacist provides comprehensive pharmaceutical patient care, adheres to department/facility/regional policies, procedures, and guidelines, communicates effectively, and demonstrates knowledge of regulatory requirements and medication safety standards.

Responsibilities

• Provide comprehensive pharmaceutical patient care.
• Follow department/ facility/regional policies, procedures and guidelines related to medication use system, pharmaceutical care and documentation of clinical interventions.
• Communicate effectively in verbal and written form to key stakeholders (peers, caregivers, patients, providers).
• Demonstrate knowledge of regulatory requirements and medication safety standards.

Requirements

• Washington Pharmacist License (1)
• Basic Life Support (BLS)- where applicable

Qualifications

• PharmD Doctorate of Pharmacy
• OrBachelor's Degree Bachelor of Pharmacy

Preferred Qualifications

• Acute care hospital pharmacy experience, 1 year or equivalent experience.
• Completion of ASHP-accredited PGY1 residency.
