About the role
Eliot’s Homeless Services Division is seeking a Clinical Outreach Navigator to join our Community Support Team. The position provides housing stabilization support to unsheltered adults who are in transition to housing pre- and post-move-in. Eliot adheres to a Housing First Model that prioritizes permanent housing as the initial step in assisting individuals experiencing homelessness.
Responsibilities
- Provides direct clinical support to assigned individuals including the provision of harm reduction counseling/education and appropriate supplies to mitigate risk of overdose
- Weekly clinical support to program staff and clients
- Consult with multidisciplinary team to support identified clients with complex behavioral health profiles, substance use disorders, and complex medical illnesses with appropriate resources and strategic interventions
- Support team in all aspects of housing placement, transition and orientation
- Responds to crisis/high risk situations collaboratively with leadership and field personnel
- Establish and maintain collaborative relationships with outside agencies
- Meet with high risk clients on the street within BOS team, in treatment programs, hospitals and houses of correction in order to assess, establish and maintain continuity of care
Qualifications
- Master’s degree in related fields
- Experience working with adults with severe and persistent mental illness or homelessness
- Ability to provide flexible services that are community based and focused on team work
- Must have a valid driver’s license and reliable transportation
Pay
Annual Salary: $70,000 USD - $70,000 USD
Schedule
Monday-Friday, 9am-5pm
