Clinical Nurse - Primary Care

About the role

Provides direct patient care in an outpatient primary care setting, including assessment, diagnosis, planning, implementation, and evaluation of nursing care according to established standards.

Responsibilities

• Develops and documents individualized care plans
• Collaborates with the interprofessional team
• Communicates effectively across the continuum of care
• Respects diverse patient preferences and promotes equity, empathy, and cultural sensitivity during care delivery
• Evaluates patient condition changes, updates care plans, and ensures evidence-based practices to achieve quality outcomes
• Participates in professional development and quality improvement activities, advocating for patient-centered care

Qualifications

• Nursing Diploma/Associate's degree
• RN license
• At least 2 years of experience

Skills

• Bachelor’s degree in Nursing preferred (with 2-5 years of experience)
• Previous outpatient or adult care experience advantageous
