Job Summary

The Clinical Manager is responsible for supervising and coordinating clinical staff, ensuring adherence to CMS Conditions of Participation, state regulations, and agency policies. This role involves oversight of patient care services, quality assurance, staff training, and compliance with physician orders. The Clinical Manager plays a critical role in maintaining high standards of patient care, staff development, and operational efficiency.

Key Responsibilities

  • Clinical Supervision & Patient Care Oversight: Ensure timely, high-quality, and compliant delivery of home health services. Supervise nurses, therapists, and other clinical staff, providing guidance and support. Review patient assessments, care plans, and physician orders for accuracy and completeness. Ensure staff compliance with OASIS documentation, PDGM requirements, and agency protocols. Conduct case conferences to review patient progress and update care plans as needed.

  • Quality Assurance & Compliance: Implement and oversee the Quality Assurance Performance Improvement (QAPI) program. Conduct chart audits to ensure proper documentation and regulatory compliance. Monitor patient outcomes, hospitalizations, and infection control data to identify areas for improvement. Ensure compliance with Medicare, Medicaid, and state home health regulations.

  • Staff Development & Training: Provide clinical education, mentoring, and performance evaluations for staff. Conduct orientation and ongoing training on documentation, compliance, and best practices. Address staff performance issues, ensuring adherence to clinical policies and procedures.

  • Operational & Administrative Responsibilities: Coordinate patient admissions, discharges, and staff assignments to maintain workflow efficiency. Collaborate with physicians, referral sources, and case managers to ensure continuity of care. Assist in the development and revision of clinical policies and procedures. Participate in state surveys, accreditation reviews, and compliance audits.

Qualifications

  • Education: Graduate from an accredited college

  • Licensure: Active state license as a registered nurse (RN)

  • Experience: 3 years of experience in home health care preferred. Management and leadership experience preferred.

  • Clinical skills as defined as generally accepted standards of practice

  • Excellent interpersonal skills and the ability to communicate effectively

  • Demonstrated proven decision-making skills

Environmental and Working Conditions

Performs duties in an office environment. If required to make patient visits, may work in patients’ homes in various conditions. Possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
