About the Role

The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is hiring a Clinical Nurse Coordinator (Psychiatric) to join the team at Connecticut Valley Hospital (CVH), Ambulatory Care Services unit. This is a full-time, 40 hours per week, first shift position, Monday through Friday, 7:00am – 3:30pm, with no weekends and no holidays.

In a state facility, this class is accountable for coordinating the various facets of clinical programs involving multiple units OR for serving as liaison between the facility and community programs and placement settings.

Role-Specific Contributions

• Serve within the CVH Ambulatory Care Services Department.
• Contribute to the Infection Control Team through active participation in infection prevention functions.
• Support pain and palliative care initiatives as part of the clinical service team.
• Provide wound care expertise and clinical oversight.
• Perform detailed data collection, analysis, and interpretation to support clinical and operational decision-making.
• Develop, format, and present clear, accurate reports based on collected data.
• Apply strong analytical skills to identify trends and help improve patient care outcomes.

Examples of Duties

• Writes clinical program objectives, policies, procedures and manuals.
• Conducts individual health assessments.
• Develops, implements and reviews individual health care plans.
• Participates or chairs individual-centered interdisciplinary teams.
• Interviews individuals.
• Accompanies individuals on visits to treatment programs or placement settings.
• Follows up on individual care in community programs to assure individuals are in the least restrictive environment.
• Assures continuity of care to prevent recidivism.
• Applies for financial aid for individuals through the Department of Social Services.
• Maintains records of individual progress or failure after placement.
• Develops community health resources and provides on-going consultation and monitoring of services.
• Acts as a resource person to staff and community.
• Prepares reports and statistical summaries.
• May clinically supervise direct care staff in execution of clinical programs.
• May develop and conduct in-service educational programs for staff and families.
• May perform recordkeeping functions.
• Performs related duties as required.

Knowledge, Skill and Ability

• Considerable knowledge of psychiatric nursing and clinical therapeutic models of individual care with emphasis on interdisciplinary team approaches.
• Knowledge of psychiatric illnesses and medications.
• Knowledge of individual treatment planning including impact of medical insurance program requirements for reimbursement.
• Knowledge of structure of services for individuals and their families in institutional and community-oriented settings, and aftercare facilities.
• Considerable interpersonal skills.
• Oral and written communication skills.

Minimum Qualifications

• Three (3) years of professional nursing experience.
• A Bachelor's degree in nursing may be substituted for one (1) year of the General Experience.
• A Master's degree in nursing may be substituted for one (1) additional year of the General Experience.

Preferred Qualifications

• Experience coordinating medical care for clients with mental health and addiction disorders.
• Experience communicating with outside medical providers regarding patient care needs.
• Experience collaborating and developing treatment plans for clients with ACS medical providers.

Special Requirements

• Incumbents in this class must possess and retain a license as a Registered Nurse in Connecticut OR hold a Multistate or Compact Registered Nurse license.
• Incumbents in this class may be required to travel.

About the Facility

DMHAS is a health care agency whose mission is to promote the overall health and wellness of persons with behavioral health needs through an integrated network of holistic, comprehensive, effective, and efficient services and supports that foster dignity, respect, and self-sufficiency in those we serve.

CVH opened in 1868 as the State's public hospital for the treatment of persons with mental illness. Today, CVH is the largest of DMHAS facilities and includes 17 inpatient units over two campuses (Middletown and Hartford). The hospital includes both general psychiatry and addiction services units. CVH provides comprehensive inpatient and outpatient psychiatric care and substance abuse treatment, including treatment for pregnant women and persons with dual diagnoses. Specialty areas include geriatric and special services, an integrated network of comprehensive mental health services for older adults and young adult services. The facility also runs the Traumatic Brain Injury (TBI) program which addresses the needs of TBI survivors, including reintegration into community living, independent living skills development, vocational and educational training, psychiatric, cognitive and neurological assessment and treatment, speech and physical rehabilitation as well as individual and family counseling.
