Responsibilities
- Support and facilitate relationships and communication with patients seeking emergency department care for primary care services.
- Link patients with a primary care physician and the appropriate community resources.
- Address barriers to accessing primary care to ensure appropriate level of care is received.
- Improve compliance with preventive care and chronic care.
- Support patient’s self-management skills for underlying health issues.
- Reduce preventable emergency department visits and/or potential hospital admissions.
Requirements
- Master of Social Work (MSW) from a school accredited by the Council on Social Work Education OR Bachelor of Social Work (BSW) with a Master's Degree in Healthcare or other related program.
- Current NYS Master of Social Work license (LMSW) or current NYS Clinical Social Work license (LCSW) preferred.
- Two (2) years of healthcare/community health/social services/human services experience.
- Five (5) years of social work experience required if BSW.
- Prior telephonic support and electronic medical record experience preferred.
Knowledge, Skill and Ability
- Strong customer service skills, and ability to quickly establish a rapport with the patient.
- Strong written, verbal and interpersonal communication skills.
- Working relationship/knowledge of health issues/barriers and the Program's service area.
- Ability to work and develop a positive rapport with patients, their families, personnel, and other agencies.
- Ability to effectively relate to patient's and life situations in a professional, supportive manner.
- Ability to facilitate linkages to appropriate community based organizations, primary care providers, programs, and services as needed by the patient.
- Computer skills, data entry experience, EMR experience preferred; willingness to learn new software applications.
- Ability to work independently.
- Ability to discern issues that need to be escalated to management.
