About the role

As a Clinical Navigator, you are the face of our hospital. You will meet people at their worst of times and, by sharing our success and the potential success they can achieve during their rehab stay, help them recognize the support needed to reach their goals.

This position works under the direction of the Director of Business Development. You will cultivate referral relationships, manage an assigned territory, and complete patient assessments. You will also make sales contacts and analyze referral and admission data from your territory.

You will collaborate with the business development team—including the Admissions Manager, admission staff, nursing, and other internal and external staff—to facilitate referral conversion. Additionally, you will provide feedback and recommendations for program development and quality improvement initiatives related to customer service, the referral and admission process, and patient and referral source satisfaction.

Developing and maintaining excellent relationships with all stakeholders—including prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives—is key to this role.

Responsibilities

  • Cultivate and manage referral relationships within an assigned territory.
  • Complete patient assessments and make sales contacts.
  • Analyze referral and admission data from the assigned territory.
  • Collaborate with the business development team to facilitate referral conversion.
  • Provide feedback and recommendations for program development and quality improvement initiatives.
  • Develop and maintain relationships with stakeholders, including patients, families, physicians, nurses, discharge planners, case managers, and payer representatives.

Requirements

  • Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
  • Minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Qualifications

  • Degree in a health-related field from an accredited college or university preferred.
  • Prior marketing and/or rehabilitation/LTACH experience preferred.
