About the Role

The Clinical Liaison serves as a link between the organization and healthcare providers, patients, and families by promoting services and helping eligible patients access care. This role focuses on building and maintaining relationships with referral sources such as hospitals, clinics, nursing facilities, and care coordinators while educating them about hospice, home health, and other supportive services.

Clinical Liaisons assist with referrals, support parts of the admissions process, and help ensure smooth transitions of care. They often meet with patients and families to explain services and answer questions, participate in community outreach and educational efforts, represent the organization at events, and track referral activity—all while maintaining professionalism, confidentiality, and alignment with the organization’s mission.

Responsibilities

  • Promote the organization’s hospice, home health, and supportive services to healthcare providers, patients, and families.
  • Build and maintain relationships with referral sources, including hospitals, clinics, nursing facilities, and care coordinators.
  • Educate referral sources about available services and eligibility criteria.
  • Assist with patient referrals and support the admissions process.
  • Facilitate smooth transitions of care for patients entering hospice or home health programs.
  • Meet with patients and families to explain services, answer questions, and address concerns.
  • Participate in community outreach and educational initiatives to raise awareness of the organization’s mission.
  • Represent the organization at events, health fairs, and professional gatherings.
  • Track and report referral activity to support organizational goals.
  • Maintain professionalism, confidentiality, and adherence to the organization’s values and mission.

Qualifications

The ideal candidate will have a background in healthcare, preferably in hospice, home health, nursing, social work, or a related field. Strong interpersonal, communication, and relationship-building skills are essential, along with the ability to work collaboratively with an interdisciplinary team. A passion for end-of-life care, patient advocacy, and community education is highly valued.
