Clinical Liaison- Hospice Sales & Education

About the role

Become a Hospice Clinical Liaison with Georgia Hospice Care. If you have a knack for coordinating, organizing, and making connections, and are ready to make a difference in the lives of others and in the community, we are seeking a dedicated Clinical Liaison.

Responsibilities

• Coordinate and generate hospice referrals within both existing and new accounts
• Identify and develop new market opportunities
• Maintain strong, ongoing relationships with hospitals, physicians, and other referral sources
• Serve as the vital link between our agency and the medical community
• Educate healthcare professionals, referral partners, and the community about hospice services
• Ensure a smooth transition of care for referred patients
• Collaborate closely with agency staff to ensure patient care is delivered efficiently and effectively
• Regularly work in the field, meeting with referral sources and partners throughout the service area

Requirements

• Education: Graduate of an accredited school of nursing
• Licensure: Registered Nurse, Licensed Practical Nurse, or Respitory Therapist currently licensed in the state
• Must successfully perform and comply with all credentialing as required by hospital/healthcare facility
• Experience: 2 years experience in a clinical care setting or hospice preferred
• Reliable transportation
• Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively
