About the role

Become a Hospice Clinical Liaison with Georgia Hospice Care. If you have a knack for coordinating, organizing, and making connections, and are ready to make a difference in the lives of others and in the community, we are seeking a dedicated Clinical Liaison.

Responsibilities

  • Coordinate and generate hospice referrals within both existing and new accounts.
  • Identify and develop new market opportunities.
  • Maintain strong, ongoing relationships with hospitals, physicians, and other referral sources.
  • Serve as the vital link between our agency and the medical community.
  • Education healthcare professionals, referral partners, and the community about hospice services.
  • Ensure a smooth transition of care for referred patients.
  • Collaborate closely with agency staff to ensure patient care is delivered efficiently and effectively.
  • Regularly work in the field, meeting with referral sources and partners throughout the service area.

Requirements

  • Graduate of an accredited school of nursing.
  • Licensed as a Registered Nurse, Licensed Practical Nurse, or Respiratory Therapist in the state.
  • Experience in a clinical care setting or hospice preferred.
  • Reliable transportation.
  • Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs.
