About the Role

The Clinical Liaison acts as a coordinator between internal and external referral sources and the acute inpatient rehabilitation operations. This role is responsible for establishing relationships with case coordinators, physicians, payors, and other referral sources. The Clinical Liaison ensures appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Coordination and the Inpatient Rehabilitation Medical Director. The role works closely with hospital Case Coordinators and the acute inpatient rehabilitation admissions team to ensure a responsive and smooth referral and admission process.

Responsibilities

  • Coordinate between internal and external referral sources and acute inpatient rehabilitation operations.
  • Establish and maintain relationships with case coordinators, physicians, payors, and other referral sources.
  • Perform clinical screenings and assessments to ensure appropriate patient placement.
  • Collaborate with hospital Case Coordination and the Inpatient Rehabilitation Medical Director.
  • Work closely with hospital Case Coordinators and the admissions team to streamline the referral and admission process.

Requirements

  • Current Clinical License.
  • Valid Driver’s License.
