Responsibilities
- Enhance patient education, post-operative teaching, medical clearance and long-term follow-up care.
- Perform and document comprehensive medical and psychosocial health histories and physical examinations.
- Provide clinical care including wound care, management of drains, chest tubes, gastrostomy and jejunostomy tubes, ostomies, central line troubleshooting, suture and staple removal, silver nitrate application to hypergranulation tissue.
- Initiate and maintain a problem/medication list.
- Order and interpret diagnostic studies including but not limited to laboratory and imaging studies, diagnostic impression, and/or form a differential based on the subjective and objective data obtained.
- Formulate a goal-directed plan that includes health maintenance, anticipatory guidance and appropriate medical and nursing interventions according to the established protocols.
- Assess and document patient responses to treatment and interventions, management and follow-up of patient's physical therapy, medication/pain management, education and other needs.
- Facilitate hospital admissions and discharges as well as coordinate appropriate in-house accommodation and home-care.
- Initiate referrals to other health care providers, and/or consult with the attending physician or the collaborating physician.
- Coordinate pre-operative medication evaluation.
- Gather, organize and correspond all information necessary for pre-certification.
- Triage patients prioritizing care based on the urgency or severity of their condition.
- Provide information on health promotion and disease prevention to patients and families.
- Review, disseminate, and incorporate research findings into clinical practice.
- Assist with child and adolescent metabolic and bariatric surgery prescreening evaluation, including surgery readiness, medical and nutritional optimization, preoperative education, long-term post-op management, and collaboration with the CABS team with appropriate referrals.
- Perform care and coordination in multidisciplinary clinics including the Esophageal Atresia clinic, Congenital Diaphragmatic Hernia Clinic, Myelomeningocele, Bowel Management Clinic and Center for Adolescent and Bariatric Surgery.
Qualifications
- Nurse Practitioners: Master's Degree of Nursing or equivalent in education.
- 3 years of nursing or related experience.
- Minimum Certification Required Upon Start Date NYS RN and NP Licenses and ANCC or AANP Board Certification.
- BLS, ACLS, PALS Certification.
- NYS DEA Certification.
