Clinical Chart Abstractor - Contractor Only (LPN/RN Required)

About the role

The Clinical Chart Abstractor will review patient EMR records to determine medical eligibility/ineligibility for Aviary Health. This role supports the enrollment team by providing clinical support and answering real-time questions.

Responsibilities

• Review patient charts for medical eligibility/ineligibility
• Create cardiac rehab referral orders when needed
• Communicate patient eligibility and ineligibility through appropriate platforms
• Respond to clinical questions from cardiac therapists in a timely manner
• Assess clinical scenarios using established protocols
• Provide clinical guidance to cardiac therapists
• Contribute to special projects as assigned

Qualifications

• 3-5 years of clinical experience in cardiac care, exercise physiology support, or chronic disease management
• EPIC or general EMR experience for 3 years
• Medical terminology knowledge for 1 year
• Proficient in computer skills, including Zoom, Google Suite, and EPIC
• Strong written and verbal communication skills
• Active RN or LPN license
• Experience with ClearTriage or similar systems
• Strong clinical assessment, triage, and analytical skills

Pay

$25 per hour

Schedule

Monday-Friday; 3-5 hours per day/15-20 hours per week; within 3pm-9pm ET timeframe
