Clinical Case Worker

About the role

New Reach is a nonprofit organization that provides shelter, housing, stabilization, and support for children, families, and individuals experiencing homelessness across Connecticut since 1990. The organization aims to inspire independence for those affected by homelessness and poverty through innovative and progressive methods.

Responsibilities

  • Afford clients with referrals to GROWW, ensuring eligibility requirements are met and assigning cases as needed.
  • Support clients in identifying unaddressed needs related to benefits, education, housing, medical, substance use, and other goals.
  • Conduct initial and annual bio-psychosocial assessments to determine service needs and complete mental health assessments with clients.
  • Work as a liaison with local mental health providers and community resources, facilitating groups with family peer support specialists or designated staff.
  • Facilitate monthly case conference meetings to collaborate with case workers and peer support specialists.
  • Provide clinical support to a caseload of single mother head of household women, linking them with mental health support or providing intermediate support under supervision.
  • Engage in case conferencing, team meetings, professional development, and community meetings as necessary to represent the Clinical team.
  • Act as a mediator between clients and landlords, addressing housing concerns such as safety, sanitation, hoarding, etc.
  • Maintain timely and accurate client files, including care plans, correspondence, and case notes.
  • Assist with special projects and other duties as assigned to ensure successful operations.
  • Participate in committees and meetings to represent the Clinical team, including the Trauma Informed Care Committee, and provide support and guidance for non-clinical case work staff.

Requirements

  • Bachelor's Degree in Human Services or related field (required)
  • Master's degree in human services field (strongly preferred)
  • 2+ years of experience in social services (required)
  • Highly motivated, creative, and excellent interpersonal skills (required)
  • Ability to provide comprehensive, intensive services to at-risk populations (required)
  • Excellent organizational skills and ability to prioritize and manage multiple projects (required)
  • Excellent written and verbal communication skills (required)
  • Excellent computer skills, including Microsoft Office (required)
  • Valid driver's license and access to reliable transportation with proof of insurance (required)
  • Bilingual/Bicultural preferred (preferred)

Skills

  • Knowledge of Trauma Informed Care, Person-Centered Care Planning, Harm Reduction, and Motivational Interviewing (preferred)

  • Effective communication and problem-solving abilities (required)
  • Ability to work independently and collaboratively (required)
  • Confidentiality and ethical standards (required)

Pay

Commensurate with experience

Schedule

Regular schedule is Monday through Friday 9:00am to 4:00pm. May occasionally flex schedule to work evenings to meet the individual needs of clients.
