Responsibilities
The CCM will also act as the Educational Liaison to support the youths' learning and to ensure their IEPs are comprehensive and meeting their needs in the school environment. This CCM will act as an advocate for the youth in the educational setting. Availability during school hours, attending school meetings, and being available during afterschool hours to establish relationships with students and provide for needs that emerge is required as well as after hours on-call support to the programs.
