Clinical Case Manager
About Us
ComoCare is a behavioral health provider built to support community health centers and the people they serve. We deliver in-person intensive outpatient therapy (IOP) to communities that have historically been underserved in behavioral health care. ComoCare partners with federally qualified health centers (FQHCs) to provide integrated behavioral health care where the patient receives whole-person care in one setting. Our team uses proven, evidence-based approaches to help individuals facing challenges such as substance use, mood disorders, and trauma — all in partnership with the trusted local health centers that know their patients best. Responsibilities
- Serve as the main point of contact for patients throughout their course of care
- Manage patient intake, admissions, engagement, and discharge processes
- Maintain an accurate and up-to-date patient census, ensuring no one falls through the cracks
- Support retention efforts by proactively addressing barriers to attendance or participation
- Monitor patient progress and collaborate with clinicians to ensure continuity of care
- Coordinate logistics and community resources to meet patient needs (e.g., transportation, childcare, scheduling support)
- Track and report on key performance indicators (KPIs) such as patient census, admissions, retention, and completion rates
- Communicate and partner with the patient’s FQHC care team, including but not limited to the patient’s primary care provider and existing behavioral health provider, to ensure each patient receives the appropriate care and resources
- Build trusting relationships with patients through empathy, consistency, and clear communication
- Work closely with therapists, call center specialists, and clinical leadership to optimize the patient experience and outcomes
This role will report to the Clinical Director.
Requirements
- 1–2 years of experience (including graduate internships/practicums) providing behavioral health services or case management in outpatient or community-based settings
- Demonstrated experience or strong interest in coordinating care with multidisciplinary teams, primary care systems, and external social service organizations
- Capability to build rapport, navigate resource delivery systems, and partner with the team to deliver trauma-informed care
- Excellent communication and documentation skills
- Strong organizational and process-management skills with attention to detail
- Demonstrated success managing patient intake, admissions, and retention workflows
- High empathy quotient with a strong commitment to patient-centered, advocacy-focused care
- Passion for serving underserved populations and advancing access to behavioral health care
Preferred Qualifications
- Experience working within community health centers, FQHCs, or similar integrated health settings
- Background working in or interning with intensive outpatient (IOP), partial hospitalization (PHP), or community mental health case management programs
- Experience using electronic health records (EHR) and specialized case management tracking systems
- Bilingual (English and Spanish)
