Role Description The Clinical Case Manager is a full-time, hybrid role based in Baltimore, MD, with a combination of on-site work and flexible work-from-home hours. This role involves providing trauma-informed case management services to program participants, including conducting psychosocial assessments, developing individualized service plans, and monitoring progress toward goals. The Clinical Case Manager will coordinate referrals to mental health, social services, and community resources, and maintain regular contact with participants through meetings, phone calls, and virtual check-ins. Day-to-day responsibilities include documenting services in accordance with organizational and regulatory standards, collaborating with internal staff and external partners to ensure continuity of care, and participating in case conferences and team meetings. The Clinical Case Manager also contributes to program development by identifying service gaps, gathering feedback from participants, and supporting data collection and reporting for grants and outcomes. Qualifications Demonstrated skills in Social Work, including client engagement, advocacy, and trauma-informed practice.Proficiency in Case Management and Case Management Services, such as assessment, care planning, service coordination, and follow-up.Knowledge of Mental Health and Social Services systems, including referral processes and collaboration with community providers.Relevant education, such as a bachelor’s or master’s degree in Social Work, Counseling, Psychology, or a related human services field.Experience working with justice-impacted individuals or populations facing complex social, legal, and behavioral health challenges.Strong written and verbal communication skills, with the ability to document accurately and maintain confidentiality.Ability to work independently and as part of an interdisciplinary team in a hybrid environment based in Baltimore, MD.Familiarity with electronic records, basic data tracking, and outcome reporting; bilingual skills are a plus.
