Clinical Case Manager

Starting at $75,000/year. Total compensation package based on experience.

About the role

AccordCare Home Health, a licensed Medicare-certified agency, is seeking a customer service–driven and detail-oriented RN Clinical Manager to join our Maitland team. This position is full-time, in-office (8:30 AM – 5:00 PM) and includes limited field case management as needed.

Responsibilities

• Perform case management, oversight, and coordination of care for all skilled disciplines.
• Coordinate patient admissions with other members of the healthcare team.
• Provide nursing care and treatment of home care patients per plan of treatment and approved authorizations.
• Supervise all field staff on assigned cases.

Requirements

• Current Florida RN License
• Level II Background Screening and CPR Certification
• Minimum 1 year of Medicare Home Health Case Management experience (required)
• Minimum 1 year of Medicare OASIS experience (required)
• Excellent time management and organizational skills
• Strong communication and customer service abilities
• Experience with Kinnser / WellSky Medicare is a plus
• Must have Medicare Home Health experience
