Clinical Case Manager

About the role

The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services. You will ensure timely access to medically appropriate care while promoting continuity, quality outcomes, and a positive patient experience.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Coordinate care across providers and service settings
• Advocate for patients and their families to ensure access to necessary resources
• Provide education to patients, caregivers, and support systems
• Monitor patient progress and adjust care plans as needed
• Maintain accurate, timely, and compliant documentation
• Deliver age-appropriate care based on developmental needs

Requirements

• Active, unrestricted Registered Nurse (RN) license in the State of Florida (must be maintained)
• 3+ years of clinical experience
• 1+ year of case management experience preferred

Preferred Skills & Abilities

• Strong computer proficiency, including Excel
• Experience with Electronic Medical Records (EMR) systems (Athena experience a plus)
• Excellent communication, organization, and problem-solving skills

Pay

The anticipated base salary range is $66,675.00 to $111,125.00. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
