About the role
The Family Bridge Clinical Case Manager / Child Placement Specialist serves as a professional child welfare practitioner responsible for applying advanced knowledge of social work, child development, family systems, trauma-informed care, and permanency planning in the assessment, placement, and ongoing support of children served through Cal Farley's Child Placing Agency.
Essential Duties
- Conduct clinical, psychosocial, and placement assessments of children, families, and caregivers to determine service and placement needs
- Develop and implement case plans, placement recommendations, and intervention strategies that promote child safety, permanency, and well-being
- Manage child placement referrals, coordinate placement matches, and facilitate timely placement of children in support, foster, kinship, or adoptive homes
- Provide ongoing case management, support, advocacy, and crisis intervention services to children, families, and caregivers
- Maintain accurate, timely, and confidential case records, assessments, reports, and professional recommendations
- Collaborate with families, schools, medical providers, community partners, and multidisciplinary teams to coordinate services and achieve positive outcomes
- Ensure compliance with child welfare regulations, licensing standards, accreditation requirements, and agency policies through ongoing monitoring and documentation
- Provide consultation, training, and guidance to caregivers, staff, and community partners on child welfare, trauma-informed care, and placement best practices
- Participate in staffing meetings, case reviews, quality assurance activities, and other initiatives supporting organizational goals and service excellence
Requirements
- Bachelor's degree in social work or a related field from an accredited college or university is required; a master's degree in social work or a related field is preferred
- A minimum of 1 year of full-time documented work experience in a child placing agency is preferred
- Professional licensure preferred, including LMSW, LCSW, LPC, LMFT, or other recognized behavioral health credentials
- 2+ years of experience working with children in in-home or out-of-home placements is required
- Proficiency in Microsoft Office and client documentation systems is required
- Advanced knowledge of child welfare principles, family systems, trauma-informed care, child development, permanency planning, and behavioral health concepts is preferred
- Experience working with foster or kinship families, schools, medical providers, and multidisciplinary child welfare teams is preferred
- Fluency in English is required; fluency in Spanish is preferred
- Must be at least 21 years of age
- Must be eligible to work in the United States
- Must possess a valid driver's license
- Must be available for occasional travel
