Case Manager - Children's Services

As a Case Manager, you will facilitate the Child and Family Team process to support youths with complex behavioral or emotional needs in achieving their optimum level of wellness.
Responsibilities
- Facilitate strategic planning using the Child and Family Team process
- Develop and implement individualized service plans for youth
- Linkage to community resources and partnerships with family members and others involved in the youth's life
- Coordinate behavioral and physical health care services
Requirements
- Master's or bachelor's degree in a behavioral health or related field
- Bachelor's degree in a non-related field plus one year related experience
- Associate's degree in any field plus one year related experience
- High school diploma or equivalency certificate plus three years' related experience
- Maintain current CPR, First Aid, and Therapeutic Options certifications
- Maintain a valid Arizona state fingerprint clearance card
- Maintain a valid Arizona driver's license and car insurance
- Be at least 21 years of age
Knowledge, Ability, And Skills
- Excellent member and family relationships and customer service
- Effective written, verbal, and interpersonal communication skills
- Organized and critical thinking, time management, and problem-solving skills
- Proficient in multiple types of office equipment and Microsoft Office software
- Utilize the internet and various web browsers

About Our Child & Adolescent Services Division
We are known for quick intake, treating even the most challenging cases, offering supportive parent and family services, and providing quality, compassionate care. We tailor our treatment to each person's strengths, needs, and culture, helping youth stay in the home, be productive in school, and have a hopeful future. We address both behavioral and physical health issues and offer special programs for teens and young adults to help them reach their full potential and become independent adults. We collaborate with the Arizona Department of Child Safety to provide comprehensive service plans for families involved in child welfare. All team members practice mindful care based on the 12 Principles of The Arizona Vision for Children.
