Case Manager Clinician - Children's Residential Program

About the role

We are seeking a dedicated Case Manager Clinician to join our interdisciplinary treatment team and help children and adolescents build the skills necessary to achieve greater stability, safety, and success in their homes and communities.

Responsibilities

  • Provide clinical and case management services to children and adolescents with developmental disabilities, autism, and significant behavioral health challenges.
  • Work as part of a multidisciplinary team, supporting individualized treatment planning, behavioral stabilization, family engagement, and successful transitions to less restrictive community settings.
  • Conduct comprehensive psychosocial assessments.
  • Provide individual and group therapy sessions.
  • Administer, score, and interpret standardized psychological assessments, including measures of intellectual functioning, adaptive behavior, and academic achievement.

  • Communicate effectively with internal teams and external partners, including CMH, guardians, and advocates.
  • Facilitate consumer transitions, including intakes, discharges, and transfers.
  • Collaborate with nursing staff to provide behavioral support related to medical appointments, including accompaniment and the collection of relevant health information when needed.
  • Participation required in on-call rotation, with varying schedules by location/program.
  • On-call pay enhancements: $50 per on-call shift weekdays, $100 per on-call shift weekends, and $150 per on-call shift holidays.

Qualifications

  • A Master's degree in Social Work, Psychology, or Counseling is required.
  • Candidates must hold or be in the process of obtaining one of the following licenses:
    • Social Work: Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW)
    • Psychology: Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), or Temporary Limited Licensed Psychologist (TLLP)
    • Counseling: Limited License Counselor (LLC) or Licensed Professional Counselor (LPC)
