Position Summary
The Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services. You will ensure timely access to medically appropriate care while promoting continuity, quality outcomes, and a positive patient experience.
How Will You Make An Impact & Requirements
- Conduct comprehensive patient assessments and develop individualized care plans
- Coordinate care across providers and service settings
- Advocate for patients and their families to ensure access to necessary resources
- Provide education to patients, caregivers, and support systems
- Maintain accurate, timely, and compliant documentation
- Deliver age-appropriate care based on developmental needs

Required Qualifications
- Active, unrestricted Registered Nurse (RN) license in the State of Florida (must be maintained)
- 3+ years of clinical experience
- 1+ year of case management experience preferred
Preferred Skills & Abilities
- Strong computer proficiency, including Excel
- Experience with Electronic Medical Records (EMR) systems (Athena experience a plus)
- Excellent communication, organization, and problem-solving skills
