About the role
NFI North is seeking a dedicated, experienced Clinical Case Manager to join our residential Transitional Housing Program in Bradford, New Hampshire. This program provides comprehensive support and services to adults living with severe and persistent mental health challenges.
Responsibilities
- Treatment planning
- Group facilitation
- Care coordination
- Skill development
- Auxiliary services such as budgeting, accessing community resources, navigating Medicaid and other benefits
Requirements
- Bachelor’s degree in psychology, Social Work or related field
- Experience working within the mental health field a plus
Qualifications
- Collaborative, organized, and strengths-based professional
- Enjoy building meaningful relationships and empowering individuals
Skills
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of mental health and behavioral support services
