About the role
Marian House is seeking a Clinical Case Manager to support women and families in our Transitional Family Program.
Responsibilities
- Provide weekly counseling and case management to families
- Develop and monitor Individual Recovery Plans (IRPs)
- Support residents with housing, healthcare, and recovery resources
- Coordinate care with internal staff and community partners
- Facilitate intake, goal-setting, and progress reviews
- Maintain accurate, confidential documentation
- Participate in team meetings and on-call rotation
Requirements
- Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field required
- LMSW, LGPC (or equivalent MD licensure), CPRS preferred
- 3–5 years of counseling/case management experience
- Experience with trauma-informed care, substance use, and co-occurring disorder
- Strong organizational, communication, and advocacy skills
Qualifications
- Preferred: Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field
