Clinical Case Manager

Responsibilities

• Perform case management, oversight, and coordination of care for all skilled disciplines.
• Cookordinate patient admissions with other members of the healthcare team.
• Provide nursing care and treatment of home care patients per plan of treatment and approved authorizations.
• Supervise all field staff on assigned cases.

Qualifications

• Current Florida RN License
• Level II Background Screening and CPR Certification
• Minimum 1 year of Medicare Home Health Case Management experience (required)
• Minimum 1 year of Medicare OASIS experience (required)
• Excellent time management and organizational skills
• Strong communication and customer service abilities
• Experience with Kinnser / WellSky Medicare is a plus
• MUST HAVE MEDICARE HOME HEALTH EXPERIENCE

$75,000/year | Total Compensation Package Based on Experience
