About the role
The mission of the Coordinator Clinical Care is to identify high risk patients and help coordinate resources to facilitate care, improve health outcomes, and to reduce avoidable hospitalizations and re-admissions. Ultimately to provide the best quality medical care available, while working toward a patient centered medical home, and to help ease our patients' way.

Requirements
- Graduate of school for medical assistants and certificate.
- Or coursework/training: Diploma/certificate from the military in Medical Specialist Course.
- 2 years directed patient care at MA level or higher.
Qualifications
Required qualifications include a graduate of a school for medical assistants and a certificate. Alternatively, coursework/training from the military in a Medical Specialist Course is acceptable. Additionally, 2 years of directed patient care at an MA level or higher is required.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
