Responsibilities
- Create care plans
- Identify gaps in care
- Track patient progress for all programs (APCM, CHI, PIN, BHI)
- Update records in eClinicalWorks (eCW)/Ecares
- Maintain accurate documentation of services provided
- Participate in quality reviews and team meetings
Requirements
- Experience 1-2 years of direct patient care experience
- Experience in Care Coordination or Case Management
- Familiarity with EHR
