Clinical Care Coordinator
About the role
This role is located in Bergen (3 days) and Essex (2 days) counties (Paramus & Bloomfield), offering onsite full-time work from 9:00 am - 5:00 pm, hourly wage of $24.04, and various benefits including health and dental coverage, union benefits, generous vacation and personal time, competitive wages, holiday compensation, 401K plan, tuition reimbursement, employee discount, life and AD&D insurance, STD and LTD coverage.
Responsibilities
- Engage consumers and their families.
- Work with the Outpatient Department to assist clients and staff with coordination of care.
- Provide linkages to community services and resources.
- Provide services based on evidence-based practices (IMR, MI and TIC).
- Ensure consumers have community supports in place and engage in service planning as appropriate.
- Communicate observations and information about the consumer in the consumer's service record and to team members.
- Participate in assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery and at discharge.
- Provide input into formulation of consumer-centered care plans in collaboration with team members, containing relevant goals, measurable objectives, and realistic time frames for achievement; perform reviews of progress and evaluate effectiveness of goals/objectives.
- Work with administrative staff to ensure that all services are documented based on billing and regulatory standards; collect information on consumers and reflect all demographic data.
Requirements
- Bachelor's Degree in Social Work, Psychology, Counseling or related field.
- Bilingual (Spanish) required.
- At least 1 year of experience in a mental health, behavioral health, medical, or another related field.
- A valid driver's license and must be insurable by agency's automobile insurance.
Qualifications
Preferred qualifications include:
- Experience in a clinical or community-based non-profit setting.
- Collaboration with social workers, clinicians, and prescribers to provide holistic and comprehensive care.
Skills
Skills required for this position include:
- Strong communication skills.
- Ability to work independently and as part of a team.
- Knowledge of evidence-based practices (IMR, MI and TIC).
- Ability to document services according to billing and regulatory standards.

$24.04 per hour.
Schedule
Full-time, 40 hours per week, Monday through Friday, 9:00 am - 5:00 pm.
