Clinical Care Coordinator

About the role

Aria Care Partners is seeking Clinical Care Coordinators to join our dynamic team. As a Clinical Care Coordinator, you will play a crucial role in ensuring that skilled nursing facilities are properly scheduled for visits according to clinical needs and efficiency.

Responsibilities

• Track provider availability and create and communicate monthly schedules for providers.
• Prepare patient lists for scheduled visits using available reports, create notifications, and send to Customer Support Specialist 2 weeks prior to the scheduled visit date.
• Maximize use of clinical days and schedule work efficiently.
• Manage requests from Customer Support Specialist to ensure residents are seen at upcoming visits in a timely manner and according to clinical need.
• Remain accessible to Providers throughout the day to provide support while providers are in clinic.
• Use metrics provided to increase efficiency in clinical day and service delivery.
• Perform other duties as necessary.

Requirements

• Education and Training: Associate or bachelor’s degree preferred – equivalent work experience accepted.
• Qualifications: Problem solving, planning/organizing, quality control/attention to detail, adaptability, dependability, computer skills, ability to lift 15-20 lbs.

Physical Efforts & Working Conditions

• Work in a normal office setting in Overland Park, KS headquarters with minimal exposure to health or safety hazards.
• Substantial time spent working on a computer.
• Position reports to the office for the first six months then qualifies to move to a hybrid schedule with successful completion of training and satisfactory performance.

Pay

Compensation is commensurate with experience.

Schedule

This is a full-time position working standard business hours Monday – Friday, with occasional project support requiring temporary adjustments to those hours as needed.
