About the Role
The Clinical Care Coordinator provides administrative support to the Clinical Care team by managing the electronic medical records system. They also support various back-office functions, including billing, payroll, human resources, and intake. Additionally, they offer training and ongoing support to new Clinical Care Coordinators.
Responsibilities
- Accurately enters a variety of data into various information systems
- Executes workflow functions in the electronic medical record system
- Verifies accuracy of data entered through collaboration with Clinical Manager(s) and/or Clinical Director
- Manages incoming, outgoing, and interoffice mail
- Provides customer support to patients and colleagues via phone, email, and fax
- Alerts appropriate management team members regarding potential compliance issues
- Maintains confidentiality of PHI and organization information at all times
- Orders DME and alerts suppliers when equipment is no longer in use or needs exchange
- Participates in and scribes IDG meetings
- Performs other specific projects relating to office procedures
- Provides training and ongoing support to new Clinical Care Coordinators within the region
Qualifications
- High School Diploma (College Degree Preferred)
- At least two (2) years of health care experience, preferably in hospice
- Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities
- HCHB/Pointcare knowledge is preferred
- Proficient in MS Office and Google Applications
