About the role
Catholic Community Services, Family Behavioral Health is seeking a Clinical Care Coordinator to join our team. We provide intensive, wrap-around mental health services to a diverse population for youth and families across multiple locations.
Responsibilities
- Provide intensive and strength-based therapeutic mental health services and supports to children, youth and families in the community
- Assess for immediate safety and stabilization needs, develop and evaluate treatment plans
- Facilitate child and family team meetings, safety and crisis planning, and ongoing assessment and evaluation
- Engage families with compassion, support, respect, and enthusiasm
- Respond to crises promptly and collaboratively, prepare families for next steps, customize helping approaches, brainstorm and negotiate strategies
- Consult with supervisors and mental health specialists, document and maintain accurate, strength-based records
- Support the implementation of the intervention plan, reevaluate and modify plans based on new information
- Facilitate placement into foster care, ensure necessary paperwork and notifications are completed
- Participate in an on-call rotation with other clinicians and coordinators
Qualifications
- Bachelor's degree in Behavioral Sciences or equivalent, accredited degree
- Energetic, innovative style with the ability to engage children, youth and family members
- Natural ability to work with others through a strengths-based approach
- Preference for a flexible schedule
- Passion for helping families stay together
- Ability to visually assess safety in the community and family homes
- Valid driver’s license and current automobile insurance, reliable transportation with an acceptable driving record
