Clinical Care Coordinator

Job Summary

The Unit Clinical Care Coordinator (Unit C3) manages patient progression through status determination, DRG and TLOS assignment, and discharge planning. This role serves as a liaison between various healthcare teams and departments to ensure efficient patient care and outcomes.

Essential Functions and Responsibilities

• Collaborates with C3s to ensure appropriate level of care and patient progression.

• Identifies and removes unit barriers affecting patient flow.

• Affirms patient status, DRG, and TLOS based on clinical presentation.

• Coordinates with UR Nurse to align payor requirements with patient progress.

• Consults and educates staff on status, placement, and resource utilization.

• Develops and arranges comprehensive discharge plans addressing patient needs.

• Led clinical care team in daily huddles to resolve patient progression and discharge barriers.

• Ensures timely updates and communication of patient status in Care Advance software.

• Collaborates with clinicians to ensure appropriate care sequencing and clinical pathways.

• Communicates patient and discharge plans to patient/family and primary care team.

• Serves as a resource on patient status, progression, and care coordination.

Qualifications Required

• Minimum of three (3) years recent acute care nursing experience.

• Bachelor of Science in Nursing preferred.

• Two (2) years of critical care experience.

• Ability to work independently, manage time, and prioritize tasks.

• Ability to maintain composure during crisis management.

• Ability to analyze information and make sound decisions.

• Clear communication skills (written and verbal).

Preferred

• One (1) year supervisory experience.

• Utilization review experience.

License and Certifications Required

• Valid RN License from Virginia or reciprocal compact state.
