Clinical Care Coordinator
Job Summary
The Unit Clinical Care Coordinator (Unit C3) manages patient progression through status determination, DRG and TLOS assignment, and discharge planning. This role serves as a liaison between various healthcare teams and departments to ensure efficient patient care and outcomes.
Essential Functions and Responsibilities
Collaborates with C3s to ensure appropriate level of care and patient progression.
Identifies and removes unit barriers affecting patient flow.
Affirms patient status, DRG, and TLOS based on clinical presentation.
Coordinates with UR Nurse to align payor requirements with patient progress.
Consults and educates staff on status, placement, and resource utilization.
Develops and arranges comprehensive discharge plans addressing patient needs.
Led clinical care team in daily huddles to resolve patient progression and discharge barriers.
Ensures timely updates and communication of patient status in Care Advance software.
Collaborates with clinicians to ensure appropriate care sequencing and clinical pathways.
Communicates patient and discharge plans to patient/family and primary care team.
Serves as a resource on patient status, progression, and care coordination.
Qualifications Required
Minimum of three (3) years recent acute care nursing experience.
Bachelor of Science in Nursing preferred.
Two (2) years of critical care experience.
Ability to work independently, manage time, and prioritize tasks.
Ability to maintain composure during crisis management.
Ability to analyze information and make sound decisions.
Clear communication skills (written and verbal).
Preferred
One (1) year supervisory experience.
Utilization review experience.
License and Certifications Required
Valid RN License from Virginia or reciprocal compact state.
