About the role
The Clinical Care Coordinator II is part of a collaborative nursing team providing day-to-day coordination of clinical activities for Dr. Ronald Lehman’s orthopedic spine practice, often across multiple sites.
Responsibilities
- Screen new patient phone calls, request medical records and imaging in advance for provider review, and assist patients with scheduling consultation appointments.
- Oversee appointment bookings and ensure emergency situations are scheduled appropriately.
- Coordinate surgical dates with attending and patient; reschedule surgeries as needed and prioritize in emergent cases.
- Provide pre-operative patient education and support, including explanation of diagnosis, upcoming testing, and surgical procedure.
- Schedule and order pre-operative testing, review completed results, and report abnormalities to the provider.
- Prepare charts and imaging for upcoming surgeries.
- Interface between providers, resident staff, nurses, therapists, and other support personnel involved in surgical patient care.
- Coordinate and facilitate communication between adult hospital, children’s hospital, and clinic facilities.
- Act as a liaison in the recovery room on surgical days, collaborating with recovery staff and provider.
- Facilitate patient transitions within and between healthcare settings (admitting, transferring, discharging, home care, visiting nurse referrals).
- Sustain knowledge of provider’s protocols and post-surgical precautions; implement and update protocols for all staff.
- Review post-operative discharge instructions with patients, including wound care and pain management.
- Round independently and/or with the provider on all orthopedic inpatients.
- Triage daily patient phone calls; provide treatment instructions and document actions in the Electronic Medical Record (Epic).
- Address patient concerns, assess and prioritize issues, and resolve or escalate to the provider as needed.
- Complete necessary forms (disability, workers’ compensation, handicap parking) and assist with patient correspondence (letters of medical necessity, school notes, insurance inquiries).
- Assist the provider during patient hours with examinations, prescriptions, and direct patient care (e.g., suture removal/application, dressings).
- Process prescription requests via telephone, send to pharmacy, and document actions in Epic.
- Prepare charts according to protocol and order x-rays for office hours using Epic.
- Contact patients and fax/email requisitions for outside radiology orders as needed.
- Review schedules prior to office hours and order x-rays for add-on patients using Epic.
- Greet and escort patients/guardians to examination rooms; ensure completion of paperwork, obtain vitals, and enter data into Epic (complying with Meaningful Use Measures).
- Communicate care plans to primary care or local physicians as appropriate.
- Act as a scribe for the provider and complete encounter forms.
- Dispose of contaminated items according to OSHA guidelines.
- Assist patients in meeting health needs by identifying problems, exploring alternatives, and supporting informed decision-making.
- Formulate patient care protocols and critical pathways.
- Assist with the development, design, and implementation of clinical research trials in accordance with hospital policies.
- Maintain patient education and instruction materials for distribution.
- Manage examination/consultation/treatment room supply inventory; track and order supplies as needed.
- Maintain medication/injection inventory and follow-up.
- Participate in professional development activities and maintain professional affiliations.
- Maintain certifications (BLS, CPR, OSHA, CLIA) and attend required training programs.
- Attend external and internal conferences, Grand Rounds, and other educational events.
- Maintain patient confidentiality and comply with HIPAA and hospital compliance guidelines.
- Attend regular clinical team and departmental meetings.
- Work as part of the orthopedic clinical team and provide cross-coverage to other staff or sites as needed.
- Manage the monthly team budget to enhance morale, environment, resources, and tools; secure and submit reimbursement forms and receipts.
- Perform scribe duties as needed.
- Other duties as assigned within scope of practice, experience, and education.
People Management
- Promote staff professionalism through coaching, training, and feedback.
- Mentor others in individual and team accountability, modeling best practices and techniques.
- Demonstrate self-development and stay current on clinical management and healthcare topics.
- Assist with interviewing, hiring, and onboarding new clinical team staff (nurses, medical assistants, front desk, administrative personnel).
- Support the physician recruitment and termination process; set up/terminate new physician offices (protocols, supplies, lab coats, prescription pads, forms).
- Coordinate monthly staff schedules to ensure coverage, track vacations/sick days, and fill posts during absences.
- Provide support to department Human Resources on staff recognition and other HR-related programs.
Additional Duties
- Oversee and document compliance with institutional policies, procedures, programs, protocols, and systems to improve practice effectiveness and patient outcomes.
- Ensure compliance with regulatory agencies, departmental, and organizational policies for all staff.
- Stay current on organizational, regulatory compliance, and patient privacy training policies (e.g., infection control, public safety, EH&S, HIPAA, OSHA) and complete required trainings.
- Ensure a safe and secure work environment by following OSHA, EH&S, and public safety guidelines.
- Maintain patient privacy and confidentiality according to HIPAA requirements.
Requirements
- Bachelor of Science degree in Nursing or equivalent education and experience.
- Minimum of 4 years of related experience.
- Current and unrestricted New York State RN license.
Preferred Qualifications
- Master’s degree.
- Physician practice experience.
- Bilingual (Spanish/English).
Pay
Salary range: $98,000 – $115,000. The salary of the finalist will be set based on factors including departmental budgets, qualifications, experience, education, licenses, specialty, and training.
