Clinical Nurse Coord

Provide specialized care to specific patient populations to optimize care and experience. Coordinate the plan of care and act as a liaison between providers, departments, other healthcare professionals, and schedulers to facilitate testing, scheduling, and referrals.

Responsibilities

  • Provides hands-on assistance according to department, patient, and provider needs, including but not limited to assisting with procedures, performing tests, preparing or administering patient medications, and obtaining specimens.
  • Act as first contact for patients, providing triaging and medical advice where applicable.
  • Coordinates the plan of care and acts as a liaison between providers, departments, other healthcare professionals, and schedulers to coordinate testing, scheduling, and referrals.
  • Monitors and manages patient’s appointments, testing, and labs necessary for the patient population.
  • Assists with obtaining prior-authorizations for the patient population.
  • Prepares educational material for patient use.
  • Educates and counsels patients concerning medical condition, medication, or treatment.
  • Provides specialized nursing assistance in research efforts when applicable.
  • Provides advice, education, and consultation services to nursing staff of any unit that care for the same patient population.
  • Attends conferences or educational opportunities to ensure up-to-date knowledge.

Requirements

Minimum Qualifications

  • Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • State criminal background check and Federal (if applicable), as required for regulated areas.
  • Obtain certification in Basic Life Support within 30 days of hire date.
  • Two years’ experience as an RN OR five years’ experience as an LPN.

Preferred Qualifications

  • Bachelor's Degree in nursing (BSN).
  • Certification relevant to the specialized care they will be providing.

Physical Requirements

  • Heavy/Hard work: Work requires strength and/or stamina, lifting, moving, stooping, reaching, positioning of patients, standing, walking, carrying of patients, materials, and equipment weighing 35+ lbs.

Working Environment

  • Frequently exposed to blood and bodily fluids; possible exposure to harmful toxins.
  • Regular exposure to communicable disease.
  • Travel may be necessary in departments with multiple locations.

Scheduled Weekly Hours: 40

Shift: Exempt
