Clinic Care Coordinator-RN

About the Role

Functions to serve as an advocate, educator, and coordinator of services for at-risk patients within Samaritan Health Services' Primary Care teams. The teams provide complete health care for all family members, from newborns to seniors, through pediatric, family medicine, and internal medicine providers, with a focus on preventive care.

Requirements

• Current unencumbered Oregon RN license.
• One (1) year of experience in a healthcare setting; two (2) years of nursing experience preferred.
• Preferred experience in:
  • Outpatient, ambulatory care, and/or community healthcare.
  • Care coordination.
  • A primary care-related setting.
  • Managing chronic conditions.
  • Electronic medical information systems.
• Bilingual skills preferred.
• For PCH Toledo Clinic: Healthcare Provider Level BLS required within 90 days of hire.
